Dermal fillers and facial contouring in Vancouver
Understanding Dermal Fillers & Facial Contouring.
Faces do not simply wrinkle as they age. They lose volume — in the cheeks, the temples, around the mouth and along the jaw — and that loss of support is often what makes someone look tired rather than older.
Filler replaces or reshapes that support. What it looks like afterwards comes down to three things: the plane it is placed in, the technique used to place it, and whether the product suits that particular area. Those are clinical judgements rather than product choices, and they are the reason experience matters here more than the name on the syringe.
That is why injectables at this clinic are performed by physicians who assess your whole face before anything is placed, in a setting equipped to manage a complication in the rare event one occurs. Placed this way, filler should not announce itself. People should simply register that you look rested.
The most common reason results look artificial is treating the line someone is worried about rather than the area causing it. We assess your whole face before we decide where anything goes.
Concerns & Goals
Concern guides connected to this treatment.
Fine Lines and Wrinkles
Visible facial creases that develop from repeated expression, collagen loss, sun exposure and skin dryness.
Perioral & Smoker’s Lines
Fine vertical lines that form on and around the lips from lip movement, aging and sun exposure.
Smile Lines & Nasolabial Folds
Creases running from the sides of the nose toward the corners of the mouth that are shaped by facial movement and midface support.
Smile Lines and Marionette Lines
Creases around the mouth, from the nose to the mouth corners and from the mouth toward the chin, that deepen with movement, volume change, and tissue descent.
Marionette Lines & Downturned Mouth
Lines running from the corners of the mouth toward the chin that can give the lower face a downturned or unhappy look.
Cheek Volume Loss
Reduced cheek fullness and downward tissue movement that can create hollows or deepen nearby folds.
Temple Hollowing
Loss of volume at the sides of the forehead that can create a sunken or aged appearance.
Facial Volume Loss After Weight Loss
Hollowing or looseness of the face that can develop after a reduction in facial fat.
Tear Troughs & Under-Eye Hollows
Hollow grooves between the lower eyelid and cheek that can cast a shadow and look like tiredness.
Under-Eye Hollows and Dark Circles
Hollowing or darkness beneath the eyes from shadowing, pigment, visible vessels, thin skin, or volume loss.
Dark Circles Under the Eyes
Under-eye darkness that can arise from pigment, visible blood vessels, shadowing, thin skin, or volume loss.
Jowls and Lower-Face Laxity
Softening or sagging of tissue along the jawline that can blur the once-defined border of the lower face.
Loose or Crepey Skin
Reduced firmness and mild tissue descent related to aging and the decline of collagen and elastin.
Chin Dimpling & Pebbled Chin
A puckered, dimpled, or pebbled chin appearance often linked to activity of the chin muscle.
Jawline Definition
A softer or less-defined jawline shaped by chin structure, facial fat, muscle size, skin laxity, or tissue descent.
Chin Projection and Facial Balance
An under-projected or recessed chin that can affect the facial profile and jawline balance.
Nose Shape, Hump & Profile Imbalance
Concerns about nasal contour, projection, symmetry, or profile balance.
Thin or Uneven Lips
Lips with reduced fullness, shape, projection, or border definition, sometimes with mild asymmetry.
Facial Asymmetry
Visible differences between the two sides of the face involving bone, muscle, fat, or soft tissue.
Gummy Smile
A smile that shows more upper gum tissue than a person would prefer.
Treatment options and technologies
Injectable filler is one of several clinically recognised approaches to volume loss and contour change in the face, and it is not always the appropriate one.
The options available here, selected according to clinical findings:
- Juvéderm · Restylane · Belotero · Revanesse — considered where volume or definition is to be restored with an immediate, reversible result, including in delicate areas such as the lips and under-eye.
- Sculptra — considered where broad, gradual support across a region is the aim rather than the shaping of a single feature.
- Radiesse — considered where firmer structural support is indicated, in areas suited to a non-reversible product.
More than one is frequently indicated, staged over several appointments rather than selected against each other.
The selection is a clinical decision, not one you are expected to make. Your face is assessed in person at rest and in movement, from the front and in profile, the areas of true volume loss are distinguished from laxity, and your medical history, medications, allergies and every previous injectable treatment are reviewed before a product or placement is recommended.

Concerns this treatment may help
Filler may be considered for:
- Flattened or hollowed cheeks
- Hollowing at the temples
- Under-eye hollows that cast a shadow
- Lines from the nose to the mouth corners, and from the mouth toward the chin
- Lip shape, definition and fullness
- Chin projection and profile balance
- A softer or less defined jawline
- Volume loss following weight change
- Differences between the two sides of the face
- Contour irregularities of the nose, in selected cases
Filler does not do everything. It does not tighten loose skin, improve surface texture or pigment, or soften lines caused purely by muscle movement. If that is what you are hoping to change, we will say so and suggest what does.
Who may benefit
Filler tends to work well for people who feel they look drawn or tired, who have noticed their face changing shape rather than simply lining, or who want one specific feature better balanced with the rest.
What matters more than age. Skin quality, how much natural support remains, previous filler and where it was placed, and how your face moves all influence what is achievable and how much is sensible.
Tell us about previous treatment. Filler placed years ago does not always behave the way people assume, and knowing what is already there — and roughly when — changes what we would add and where.
When we would advise against it. If skin laxity rather than volume loss is the real issue, filler will not fix it and adding more will make the face look heavier. Clear goals matter here; if what you want is not achievable with filler, hearing that at consultation is far better than finding out afterwards.
Your consultation
Filler is injected here by a physician — Dr. Gidon Frame, Dr. Sarina Govindasamy or Dr. Teri Fisher — and the doctor who assesses you is the one who treats you.
We look at your face at rest and in movement, from the front and in profile, because contour problems are often more obvious from the side than in a mirror.
We ask about your medical history, allergies, autoimmune conditions, medications including blood thinners, any recent or planned dental work, and every previous injectable treatment you have had.
We also talk about what is realistic. That includes which product would be used, why that area rather than the one you may have expected, how long it should last, and what would happen — and who you would call — in the rare event of a complication.
Asking costs nothing: the consultation itself is complimentary. What you pay at the time of booking is a fee that secures the appointment, reserving that block of the physician's day and cutting down on appointments left empty.
What treatment is like
Before we start. The area is cleaned thoroughly. Numbing cream may be applied, and most hyaluronic acid products contain local anaesthetic within the gel itself.
Needle or cannula. Some areas suit a fine needle; others are better treated with a blunt-tipped cannula, which moves through tissue rather than piercing it. The choice depends on the region being treated.
Small amounts, then reassess. Product is placed gradually. You may be asked to sit up, smile or turn your head partway through so both sides can be compared properly.
What you may feel. Pressure, a brief sting, and sometimes an odd sensation of movement. Most people find it far more manageable than they expected.
Afterwards. You can leave straight away. Expect to look a little swollen, and do not judge the result on day one.
Results and treatment planning
What filler can improve: facial volume and contour, projection and balance of features, shadowing caused by hollowing, and the softening of folds that come from lost support.
What it cannot do: lift significantly loose skin, improve skin texture or pigment, stop ageing, or last indefinitely. It is a restoration of shape, not a change to the skin itself.
When you see the change. Hyaluronic acid gives an immediate result, though early swelling exaggerates it — the true outcome shows once that settles over the following weeks. Collagen-stimulating products are the opposite: little to see at first, with change developing over months.
Series and maintenance. Most areas are completed in one visit, but collagen-stimulating treatment is planned as a staged series. All fillers are temporary and gradually break down, so maintaining a result means repeating treatment over time.
What influences how long it lasts: the product used, the area treated and how much it moves, how much was placed, your metabolism, and your age and activity level. Mobile areas such as the lips typically break product down faster than structural areas like the cheekbone.
Faces are not symmetrical to begin with, and a good result respects your face rather than trying to make both sides match exactly.

Recovery and aftercare
Most people return to normal activity immediately, but plan around swelling and the possibility of bruising if you have an event coming up.
What to expect: swelling and tenderness over the first days, sometimes bruising, and a firmness in the treated area that softens as the product settles. Lips swell more than anywhere else and can look startling for a day or two.
What helps: cool compresses, sleeping with your head slightly raised, and leaving the area alone.
What to avoid for a short period: strenuous exercise, saunas and hot yoga, alcohol, facial massage or facials, and pressing or moulding the area yourself.
Contact the clinic immediately if you develop severe or increasing pain, skin that turns white, blotchy, dusky or unusually mottled, or any change in your vision. These are uncommon but need urgent attention, not a wait-and-see approach.
Common and usually temporary effects
Injection-site reactions are expected and usually settle within days:
- Swelling, particularly in the lips and under the eyes
- Bruising, which can occasionally last a couple of weeks
- Redness and tenderness at the injection points
- Firmness or small palpable lumps as the product settles
- Mild asymmetry in the early days while swelling is uneven
- Itching or a sensation of tightness in the treated area
Less common but important risks
Filler is a medical procedure and carries real risks, some of them serious. Less commonly it can cause:
- Vascular occlusion. Filler entering or compressing a blood vessel can restrict blood supply to the skin, and in rare cases affect the eye. This is the most serious risk of filler and the reason placement, depth and technique matter so much.
- Persistent lumps or nodules, which may appear soon after treatment or, less often, months later.
- Infection at the treatment site, or reactivation of a cold sore where treatment is near the mouth.
- Lasting asymmetry or an over-corrected, unnatural appearance.
- Delayed inflammatory reactions, sometimes triggered by illness, dental work or vaccination.
- Allergic reaction to a product or its components.
- Migration or displacement of product from where it was placed.
- Skin damage or scarring, usually as a consequence of an untreated vascular problem.
Warning signs to act on straight away: pain that is severe or getting worse rather than easing, skin that goes white, blotchy, dusky or mottled, unusual coolness in the area, or any visual disturbance. Contact the clinic immediately — early treatment makes a substantial difference to the outcome.
Being treated by a physician who knows facial anatomy and can recognise and manage these complications is the single most important safety factor in filler treatment.
When treatment may need to wait
Filler may need to be delayed or avoided if you have:
- Active infection, inflamed skin or open lesions in or near the treatment area
- An active cold-sore outbreak, where treatment is around the mouth
- A history of serious allergic reactions or anaphylaxis
- Certain autoimmune or connective tissue conditions
- A bleeding disorder, or current blood-thinning medication
- Recent or upcoming dental work, vaccination, or a current illness
- Pregnancy or breastfeeding
- A known sensitivity to a filler product or to lidocaine
- Untreated skin disease affecting the area
- Goals that filler genuinely cannot achieve
This is not a complete screening list. Your medical history, medications, allergies, previous injectable treatments and the anatomy of the area are all reviewed before treatment.
Your at-home skincare
Filler and skincare address different layers, and neither substitutes for the other.
Filler restores shape beneath the skin. It does nothing for tone, texture, pigment or fine surface lines — that is where a sensible routine and daily sun protection earn their place. Skin that looks healthy makes a filler result look better, because the light falls more evenly across it.
Sun protection also matters for a less obvious reason: it helps preserve the collagen and elastin that give your face its own remaining support, which is exactly what filler is compensating for.
What you use at home is part of the treatment, not an extra. We carry a full range of medical-grade skincare at the clinic, and deciding what your skin actually needs is part of your consultation. Medical-grade formulations differ from most retail skincare in the concentration and stability of their active ingredients, which is why they are recommended after an assessment rather than chosen from a shelf.
For most people the categories that matter are a cleanser suited to your skin, an antioxidant in the morning, an active such as a retinoid at night where it is appropriate for you, and daily sun protection. See the skincare range, or ask at your appointment.

FAQ
Questions about Dermal Fillers & Facial Contouring.
They should not, if the amount and placement suit your face. Filler looks obvious when too much is used, when it is placed in one area without regard for the rest of the face, or when it is layered repeatedly without accounting for what is already there. Building gradually and assessing the whole face is what keeps a result quiet.
The consultation itself is complimentary, so there is no cost attached to simply asking whether filler suits you. A booking fee is taken to secure the appointment time with the physician, which is how we keep the schedule reliable for everyone. Our reception team explains it when you arrange the visit.
Because that fold is often a shadow cast by the cheek above it losing support. Filling the crease directly can make the area look heavier and more pronounced, while restoring the cheek lifts the tissue that was causing the fold. Treating the cause rather than the symptom generally gives a more natural result and uses product more efficiently.
It depends heavily on the area. Lips swell the most and can look dramatic for a day or two before settling. Cheeks and jawline are usually more discreet. Bruising is unpredictable — some people bruise every time and others never do — so if you have something important coming up, leave a comfortable margin rather than booking close to the date.
Filler at a sensible volume replaces support that has been lost rather than stretching tissue. The concern is real when very large amounts are placed repeatedly over years without reassessment. This is one reason we ask what filler you have had before and how recently, and prefer to build conservatively rather than adding to an area that already has more than it needs.
Hyaluronic acid filler can be broken down with an injected enzyme, usually over one or more appointments depending on how much is present and where. The area may look somewhat deflated afterwards while tissue settles. Poly-L-lactic acid and calcium hydroxylapatite products cannot be dissolved this way — they resolve over time on their own, which is why product choice deserves thought upfront.
Yes. Dental procedures, vaccinations and infections can occasionally trigger a delayed inflammatory reaction around existing filler, sometimes months after treatment. It is uncommon and generally manageable, but knowing your timing lets us schedule sensibly and recognise the cause quickly if a reaction does occur.
Often yes, and combinations are common because filler addresses volume while other treatments address movement, skin quality or laxity. Sequencing matters though — some treatments are better done before filler and others afterwards, and certain combinations are deliberately spaced. Your physician will set the order so nothing interferes with anything else.
It deserves the caution. The skin there is thin, swelling shows readily, and the anatomy is unforgiving of product placed at the wrong depth. Not everyone with under-eye shadowing is a candidate — if the darkness is from pigment or visible vessels rather than hollowing, filler will not improve it, and sometimes restoring the cheek beneath gives a better outcome than treating the hollow itself.
Call the clinic rather than waiting to see whether it settles. Pain that is worsening instead of easing, skin turning white, dusky or mottled, unusual coolness in the area, or any change in vision all warrant immediate contact. These situations are rare, but they respond far better to prompt treatment, and we would much rather hear from you unnecessarily than late.
